Nursing: Excessive Use of Physical Restraints

Evaluation plan

Taking into account the negative effects of the excessive use of the physical restraints on the patients’ well-being, training of the nursing staff is required for exploring the existing alternatives and reducing the use of the restraints to the absolute minimum. The effectiveness of the proposed solution for the discussed problem depends upon the consistency of the training program for motivating nurses and guiding them in making professional decisions in every individual case.

Methods

The methods which can be used for evaluating the effectiveness of the training program include obtaining the rates of using the physical restraints before and after the participation in the program, survey of staff attitudes towards the initiated change, and their motivation for reducing the physical restraining to the absolute minimum a considering the existing alternatives and survey of patients and their relative’s attitudes towards the use of physical restraints in certain cases (Cubbin 2000).

The combination of qualitative and quantitative research methods allows assessing the impact of the training program upon nursing staff, patient population, and treatment outcomes. Comparing and contrasting the rates of using the physical restraints before and after initiation of the change is significant for measuring the impact of the program on the nursing practice, but would be insufficient for defining the underlying cause and the psychological coloring of the discussed modification.

A Survey of the nurses’ attitudes towards the implementation of the alternative methods whenever it is possible is needed for evaluating the long-lasting consequences of the changes. The analysis of the patient attitudes is crucial for assessing the outcomes as the results of the training program.

Variables

The variables which can be measured in the course of this evaluation plan include the nurses’ attitudes and perceptions of the initiated change, patients’ attitudes and perceptions of the reduced use of physical restraints, and the rates of use before and after the training program and its negative consequences for the patients’ well-being.

These variables are intended to demonstrate the changes in the nursing practice associated with the implementation of the proposed solution, thus, evaluating its effectiveness in general. Enhancing nurses’ understanding of the negative consequences of the excessive use of physical restraints and exploring the existing alternatives for guiding the nursing staff in making decisions concerning the appropriateness of using the restraints in every individual case is expected to change their attitude and reduce the use of restraints to the absolute minimum, consequently decreasing the cases of the negative effects of these strategies (Pearson, Vaughan, & Fitzgerald, 2005).

Though the patients’ and their relatives’ attitudes towards the initiation of the change do not show the direct results of the training program, their satisfaction with the quality of the services is a significant parameter that needs to be taken into consideration for measuring the outcomes.

The statistics data, namely the rates of the use of the physical restraints and the corresponding data on the negative consequences of the implementation of the method or its alternatives in every individual case is needed for evaluating the scales of the changes as the proposed solution for the existing problem (Huang, Chuang, & Chiang, 2009, p. 241).

Using the qualitative research methods for evaluating the nurses’ and patients’ attitudes and perception of the reduced practices of using the physical restraints and the qualitative methods for evaluating the rates of the use will help evaluate the effectiveness of the proposed solution.

Disseminating Evidence

For disseminating the results of the project to the greater nursing community and key stakeholders, detailed information on the methods and outcomes of the experiment needs to be included in the professional conference materials and published in a professional journal.

Announcing the results of the experiment at a meeting with key stakeholders would demonstrate the benefits of the evidence-based approach for the nursing practice stimulating funding of the following research and disseminating evidence to the greater nursing community. Along with preparing a report for a specialized conference, the researchers should allocate the costs for printing the materials and sharing the valuable experience of the experiment and its outcomes.

Focusing on collaboration among the nursing community, it is important to mention the limitations of the current study and the opportunities for further research in the report on this study. Disseminating the results of the project along with the implemented methods and strategies is an important element of the research process.

Sharing the valuable experience of training the nursing staff for changing their attitudes towards the use of existing alternatives instead of physical restraints and improving the outcomes of the intervention strategies would allow contributing to the existing nursing theories and improving the outcomes of the intervention strategies in the hospital settings. Guiding nurses in making the appropriate professional decisions in every individual case would have a positive impact upon their working conditions and treatment outcomes.

Disseminating evidence to key stakeholders and the greater nursing community would enhance the effectiveness of the experiment and stimulate further research and improvement of the training program as the proposed solution for the problem of the excessive use of physical restraints in hospital settings.

Appendix 1

Questionnaire for nurses

  1. Do you consider reducing the use of physical restraints important for the hospital setting?
  2. Do you consider alternatives to physical restraints in each situation?
  3. Can the alternatives be as effective as physical restraints for preventing the patients’ injuries?
  4. How did the reduced number of physical restraints affect your working conditions?

Appendix 2

Questionnaire for patients and their relatives

  1. Are you for or against the use of physical restraints for preventing the patients’ injuries?
  2. Can the alternatives be as effective as physical restraints for preventing the patients’ injuries?
  3. What is the impact of the reduced number of physical restraints upon the quality of healthcare services?
  4. How did the reduced number of physical restraints affect attitude to hospital setting?
